These attendance forms will need both the parent/guardian and student signatures before being brought to the front office at Presentation Academy.

Reporting an Absence/Tardy: Please call (502) 583-5935 before 9 a.m. if your daughter will be absent or tardy. Please provide the student name, brief reason for illness/tardy and person calling. A parent note is always required for any tardy or absence.

Student Services Policy: It is our policy not to deliver phone messages, lunches/lunch money, sports equipment or homework to students during class unless it is an emergency, to be determined by the administration as it is a disruption to class. Any items dropped off for students may be picked up in the front office after class.

​In order for school personnel to administer any type of medication to your daughter at school, we must have on file a signed affidavit giving us your permission. The medication should be sent to school with complete instructions and in its original container with the original prescription label attached. Please be sure to complete all information that is listed on the form before returning it to school.

This form should be completed for each prescription and/or over-the-counter medication (cough drops, aspirins, etc.) that your daughter must take during school hours.
